The Importance Of Twice-Yearly Visits​

It is recommended that patients have a minimum of two primary care visits each year, with more appointments recommended for patients with chronic conditions. By maintaining regular visits, we are better able to assess patients. During these appointments, patients provide their doctors with valuable insights about their health status and bring up symptoms that may indicate the presence of more serious issues that could lead to the need for complex – expensive – interventions.
Proper documentation during these visits is central to determining the health of the patient. In this context, the medical record for each patient encounter should include the “MEAT” criteria, which refers to the following information:

M – Monitoring disease progression, disease regression, signs of disease, and symptoms of disease

E – Evaluating medication efficacy, patient response to treatment, and test results
​

A – Assessing/Addressing may include counseling, discussion, ordering tests, and reviewing records
​

T – Treating with the appropriate modalities, such as medications and therapies​
